Basics
- Place Project Record Book securely in a 4-H folder from your county Extension office.
- In the upper right hand corner of the outside folder cover, please include the following information: Name, County, District, Project Record Category (ex. Animal Science), and Age Category (8-10,11-12,13-15,16-18). A label sticker is great for this.
- Please be sure to complete all of the information requested on this page.
- Please print legibly or type using Arial or Times New Roman font in size 12 ONLY.
Section 1:

- Make sure the appropriate people and you sign your Project Record.
- Make sure your age category is as of January 1st.
- For Show & Sale and Chick Chain the Category is Animal Science
- Make sure to answer, "Why?"
- There is always room for improvement! Be sure to answer the Looking to the future section.
Section 2:
- Tell us your 4-H story! Your story should be no more than 4 pages. It can be typewritten or printed neatly. It should include your entire 4-H experience for this year. Introduce yourself to the judge.

- Introduce yourself and include any information you choose to share about your interests, family, hobbies etc. If you were given two minutes to tell someone about yourself, what would you say?
- Describe how 4-H has made a difference in your life.
- How did the project you are completing in this Project Record help you grow?
- Who has helped you in your 4-H journey?
- How has your 4-H participation helped others?
- What are your goals in life?
- Summarize your experiences in 4-H.
Section 3:
- We "learn by doing" in 4-H. List the specific things you learned and did in this project area this year.
- List knowledge and skills gained as well as the things you learned how to do.
- Include dates and levels of participation (club, county, district).
- Please list the most important presentations, talks, exhibits, radio, TV, newspaper articles, tours, camps, judging events, workshops, and any other learning experiences in which you have participated related to your project.
- The 3 most important things to do: list things you learned and did in this specific project, show the progress from start to finish in this specific project, show dates and levels of participation?
Section 4:
- List leadership roles you took in 4-H or other activities (e.g., club officer, committee member).
- Detail any community service or citizenship activities you participated in. Highlight how you helped or taught others in 4-H. Include any public speaking or mentoring experiences.
- Provide dates and levels of participation (county, district, state, national).
Section 5:
- Document any awards, honors, or recognitions you received this year.
- Include ribbons, trophies, scholarships, or certificates. Please attach ribbons securely.
- Mention competition results or achievements related to 4-H.
- Keep entries concise but detailed enough to show accomplishments.
Section 6:
- Focus on Your Project – Make this the main part of your section with details, photos, and highlights.
- Showcase Your Work – Use pictures, drawings, certificates, or news clippings (with captions).
- Include Other 4-H Activities – Keep this brief, just a short paragraph or two.
- Follow Guidelines – Six-page max, no items thicker than 1/8 inch, protective sheets optional.
- Digital Media – Max 2 minutes, upload to YouTube, include closed captions (required).
- Stay Organized – Use headings, bullet points, and clear formatting for easy reading.
Section 7:
- Be Specific – Say what you want to do, how, and when you'll do it.
- Use the Guide – Start with "I want to...", choose an action, a result, and a timetable.
- Mix It Up – Set goals for projects, leadership, community service, and personal growth.
- Break It Down – Big goals? List small steps to reach them.
